We provide you with all the necessary supplies including fixatives, containers and biopsy request forms pre-printed with your name on them. Pap testing supplies include Pap-paks, or blank slides and slide folders, cytology fixative, cervical scrapers, cytobrushes and cytology request forms, Thin Prep® pap test™ supplies. For physicians that are not local we supply postage-paid mailing containers.

SHIPPING INSTRUCTIONS

1. Insert labeled specimen(s) bottles and completed requisition(s) into a crush proof cylinder.
2. Securely fasten the metal lid to the cylinder. Put closed cylinder into Express Mail envelope.
3. Complete the ‘FROM’ portion of the mailing label with name, address and phone number.
4. Remove the back portion of the mailing label and apply it to the Express envelope.
5. Keep the ‘customer copy’ of the mailing label for your records. This is a way to track the envelope in case of an incident or problem with shipping.
6. Place express envelope in express drop box or give to your local carrier.
